1) Print your own form: Click here. A membership form will appear in a new window. Print it on your printer (just close the second browser window when finished), fill it out, and mail it with your cheque or money order payable to Domino Theatre Inc. to

2) Use the form in the Domino playbill: Fill out the membership and subscription form in the Domino Theatre playbill and mail it or deliver it with your cheque payable to the Grand Theatre to the Grand Theatre box office at the address shown on the form.
